Free E/M Level Calculator
Paste a progress note. Get the office visit E/M level, new or established, and the complete medical decision making breakdown behind it - so every level you bill is one you can defend.
De-identify, then paste
The progress note for one office visit, with the patient's name, dates, numbers and other identifiers removed.
We read the documentation
Problems addressed and their status, data reviewed and analyzed, the risk of the management chosen, and documented time.
Get the level and the why
The E/M code, each element's level, and the exact words in the note that support it.
What you get
- The E/M level for a new and an established patient (99202–99205, 99212–99215)
- Medical decision making: problems, data and risk, each with its level
- Every problem addressed, with its status today and the supporting documentation
- Data reviewed and analyzed, by AMA category
- The risk of patient management, with what drove it
- The time-based level when total time is documented
De-identified notes only
The free calculator is not covered by a Business Associate Agreement, so please use a de-identified or synthetic note. Notes that still show common identifiers are refused. The note text is deleted as soon as the calculation finishes, results are deleted after 30 days, and nothing is ever sold, used for advertising or used to train any system.
How the E/M level is set
Office visit levels follow the AMA medical decision making guidelines: three elements, and the level is the highest one that two of the three reach.
Problems addressed
How many problems the provider evaluated or treated at the visit, and how serious and stable each one is.
Data reviewed and analyzed
Tests ordered or reviewed, outside records, history from someone other than the patient, independent interpretation and discussion with another clinician.
Risk of management
The risk of the care chosen at the visit, from rest and fluids up to decisions about surgery or hospital care.
Each element is rated, and the visit's level is the highest one that two of the three reach. When the provider documents total time on the date of the visit, the level can be set by time instead; the calculator works out both and reports the higher one the note supports.
Explained in our own words. For the authoritative definitions, see the AMA's CPT® Evaluation and Management guidelines; to try any mix of the three elements, use the MDM level picker.
A worked example
A synthetic visit, for illustration.
Established patient with sore throat and fever for two days. Rapid strep done in office: positive. Amoxicillin prescribed.
Questions
Is the E/M calculator free?
Yes. Sign in with your work email and your first three calculations are free.
Can I paste a real patient note?
Only after removing the identifiers. The free calculator is not covered by a Business Associate Agreement, so it is meant for de-identified or synthetic notes. A note that still shows a labelled patient name, date of birth, phone number, email, record or account number, Social Security number or street address is refused before it is processed.
Which visits does it cover?
Office and other outpatient evaluation and management visits, new (99202–99205) and established (99212–99215) patients. It does not level emergency department, hospital, nursing facility, home or consultation services.
Does it use medical decision making or time?
Both. The level is set by medical decision making - two of the three elements - and, when the note documents the provider's total time on the date of the visit, the time-based level is calculated too. The higher supported level is reported.
What if I don't know whether the patient is new or established?
Choose "Don't know" and you get both codes for the level the documentation supports.
What happens to the note I paste?
It is used only to calculate your result. It is encrypted in transit and at rest, and the note text is deleted as soon as the calculation finishes. The result is kept for 30 days so you can reopen it, then deleted. Nothing is sold, used for advertising or used to train any system.
Can I bill the level it shows?
The result is a recommendation based only on the documentation you provide. Final code selection remains with you and your coding team.
